Plum Health DPC, or Direct Primary Care, is a relatively simple concept: $49 per month for as-needed healthcare, lowered presciption costs and lab tests. The doctors are also available 24/7 and make housecalls.
Dr. Paul Thomas and Dr. Raquel Orlich are both Michigan natives who wanted to be able to treat their patients without the insurance middle-man, so Thomas started the clinic and hired Orlich to work with him. They make their profits off membership dues, but everything else is at cost.

I can't describe how much I love this concept. Pat and I have terrible insurance with a high deductible; I'd absolutely make the drive tor $49 per moth.
